Please print the “Welcome” form and fill out each section as accurately as possible. This contains important diagnostic information for the doctors and staff so that your visit will be very thorough and efficient.
Health Insurance vs. Vision Insurance
There are two types of insurance that are accepted at our clinic: Health Insurance and Vision Insurance. As a general rule, if you are having a medical problem (conjunctivitis, retinal detachment, dry eye syndrome) with your eye, this would be billed to your health insurance. If you are coming in for a routine eye exam, this would be billed to your vision insurance.
